Hepatology Digest: Congratulations on receiving the "World Outstanding Chinese Physician Fok YingTung Award". What are your thoughts on this honor, and what does it mean to you? Looking back on your career, what clinical observation or patient experience prompted you to shift from solving individual patients’ problems to designing research that transforms populationlevel clinical practice? How do you understand the relationship between "clinical excellence" and "research innovation"?

Prof. Anna S.F. Lok: First of all, I am deeply honored to receive this award. This honor recognizes my decadeslong work, and it would not have been possible without the support of many colleagues, whose help has shaped my professional career.

It was not any single patient story that inspired my research. My decision to focus on hepatitis B stems from the huge burden this disease places on people in China. Early in my career in Hong Kong, I witnessed many patients suffering from hepatitis B-related cirrhosis and liver cancer in their prime years. This drove me to gain a deeper understanding of the disease so that I could deliver better care.

As clinical researchers, we work closely with patients, and many of our research ideas originate from clinical encounters. This sense of responsibility motivates us to keep exploring and seeking better treatment strategies. The primary duty of a good physician is to care for every individual patient but helping individual patients has limited reach. Through clinical research, if we can identify better therapeutic strategies, we can help thousands and millions of patients, including those we will never meet in clinic. Therefore, clinical practice and research complement each other. In clinical work, we help patients one at a time — and this is critically important, because every patient is an individual who deserves our full attention. Research innovation, by contrast, magnifies clinical insights to benefit far larger populations.

Hepatology Digest: Clinical excellence demands unwavering commitment to patients, while research innovation requires questioning existing evidence and accepting uncertainty. Amid heavy clinical, teaching and administrative responsibilities, how do you identify researchworthy clinical questions and translate them into feasible study designs? At the University of Michigan, how do you train young hepatologists to accomplish this transition from clinical practice to research?

Prof. Anna S.F. Lok: There are countless things we wish to accomplish, yet we only have 24 hours each day. We therefore need to define core priorities, sort out what matters most, and stay wellorganized to manage our time and workload. We constantly strive to balance clinical care, teaching, mentorship and research.

In research, the most critical step is to identify major unmet clinical needs, then develop efficient, scientifically sound and practical study designs, and move the projects forward through structured planning and consistent effort. Meanwhile, teamwork underpins research breakthroughs. A strong team enables highquality work on a larger scale, which requires talent development and mentorship.

When mentoring young physicians, I adopt a studentcentered approach. I seek to discover each trainee’s strengths and interests, and encourage them to pursue topics they are genuinely passionate about. Passion drives young investigators to devote time and energy to indepth exploration, and when research hits roadblocks, that same passion sustains their motivation and keeps them from giving up.

It is unrealistic to accomplish everything all at once when juggling multiple responsibilities. We must assess urgent priorities, draw clear work plans and maintain good organization. We also need to foster proper division of labour within the team, so that members can perform their respective roles collaboratively.

Hepatology Digest: Many bottlenecks arise for frontline clinicians as they grow into physicianscientists with both clinical competence and research output. In your view, what core competencies are most essential for young hepatologists to develop?

Prof. Anna S.F. Lok: To me, the cornerstone of young physicians’ professional growth is to clarify career aspirations, set concrete goals, and formulate actionable plans. They need to implement plans stepbystep and persist in the face of challenges. A good mentor can provide professional guidance, comprehensive support and even necessary research resources to help trainees stay on course and reach their goals.

I often remind young people that success in medicine never comes easy. It can only be achieved through sustained hard work. When confronted with setbacks in research or in clinical practice, they should not give up halfway. By staying true to their original aspiration and persevering, they will overcome bottlenecks and reach their professional goals.
